Discharge From the Hospital
Discharge
Your health care team will collaborate with you and your family on the optimal time for you to go home or to another appropriate care setting. You may receive prescriptions for medication, special instructions, and/or a follow-up appointment with your doctor or other health care professionals.
On your day of discharge, please make arrangements for a family member or friend to pick you up on or before 11:00 a.m.
Please make sure you pay for any charges you may have (ambulance bill, etc).
Outstanding invoices can be paid in person at either hospital site, by cheque, cash, debit or credit card. Payments can also be made by mailing in a cheque (please reference the patient ID# or invoice #).
Discharge Planning
Your health care team will work toward your discharge home from the moment you are admitted to the hospital. The health care team is multi-disciplined and is comprised of your physician, nursing staff, physiotherapist, occupational therapist, dietitian, diagnostic imaging staff, laboratory staff and discharge planning staff.
At Muskoka Algonquin Healthcare, discharge planning is carried out in partnership with Care Coordinators from Ontario Health atHome. Care Coordinators connect you with the care you need at home and in your community. They support an individual getting the care needed to stay well, heal at home, and stay safe in their home longer.
Home care services are publicly funded and are at no cost to those with a valid Ontario Health Card.
