Heart Function Clinic

The Heart Function Clinic offers outpatient care, routine management, and education to patients with congestive heart failure. Under the care of our Specialized Cardiac Nurse Practitioner and Case Manager, with an escalation pathway to access a Cardiologist if needed, the clinic provides ongoing medical assessment and follow-up through in-person and virtual appointments. 

Our services include:

• Evaluation and monitoring of heart function 
• Support with symptom management 
• Medication monitoring and dosing
• Ongoing diagnostic testing, such as bloodwork and ultrasound, and referrals for additional care as appropriate 
• Education about lifestyle changes and dietary requirements for living with congestive heart failure 
• Palliative care support options as required 

Referrals & Appointments

Heart Function Clinic appointments are made upon referral by a primary care provider.  MAHC will contact you to schedule your appointment and you will receive all of the information you need to prepare for it.  In some cases, we are able to accommodate virtual appointments. If this is something you may require, please let us know at the time of booking.  Should you need to reschedule or cancel your appointment, please email heart.function@mahc.ca or call the clinic at 705-645-4404 ext. 2378.

Please register for your appointment at Patient Registration and attend the Outpatient Clinic next to the Emergency Waiting Area on the first floor.

The Heart Function Clinic operates Mondays: 8:00 a.m. - 4:30 p.m. (note that hours may vary)
