Complex Continuing Care

Patients receive highly specialized medical services within both of our hospitals, for long-term illnesses or disabilities when their needs can't be met in the community or long-term care.  Muskoka Algonquin Healthcare is part of the regional Complex Continuing Care program.  Non-Palliative patients are referred to this program before admission and may be admitted to one of two locations within the region.  Please refer to the brochure for additional information.

Referrals

Application and access for admission to Complex Continuing Care beds is facilitated through the North Simcoe Muskoka Ontario Health atHome care team. The Care Coordinator or physician will discuss the option of complex continuing care with you, when you are ready to be discharged from acute care or admitted from home.  If complex continuing care is the best option for you, your Care Coordinator will refer your case to Ontario Health atHome.

Admission

If this program is the best choice for you, your name will be added to a regional wait list. You may be admitted to a complex continuing care bed at any one of the following locations:

  • Muskoka Algonquin Healthcare (Bracebridge)
  • Georgian Bay General Hospital (Midland)

You will be notified when a bed is available for you at one of these hospitals.  The hospital will assist you with your transfer to the site, and help you with all of the information you need.  When you are admitted, the care team will monitor and assess your progress.  They will also help you to make plans for discharge.  Your hospital stay should be from two weeks to three months.  If your stay is longer than three months, you may choose to transfer to a complex continuing care bed at a site of your choice, once a bed is available.
